JR played nice to Babe and Josh in the nursery, claiming he was there to take his son to the park, but Babe told JR he wasn't going to take Little Adam anywhere. She tried to throw Josh out; Josh spat that JR would only try to kill her again, then exited. Little Adam went off to have dinner, leaving his folks alone in his room. Babe accused JR point-blank of having tried to kill her and swore he wouldn't get away with it. He tried to deny it, but she ticked off the evidence she'd accrued and he stopped defending himself. She asked if she should call the police. JR admitted to Babe that he loved her when he married her, but didn't believe her when she said the sentiment had been mutual. Babe acknowledged that she had, at one time, intended to snatch custody of Little Adam from JR, but that she had genuinely fallen back in love with him along the way and scrapped that plan. JR had his doubts. Babe said she didn't want to turn him into the police; she wanted him to help her fight for his survival and the survival of their marriage. She asked if he was willing to seek help. The answer depended, he implied, on what kind of help she was talking about. She wanted him to check into a substance abuse program to save their family. "Will you fight for us?" she asked, tears streaming.

Amanda tracked Jamie down during a study session and laid a kiss on him. A chipper Amanda explained that she was going to leave town for a while to serve as a surrogate mother. He had strong reservations. Josh interrupted their chat to talk to Jamie; Amanda was happy to leave. Josh reported to Jamie that Babe is in trouble because Jamie's lunatic brother wants her dead. Jamie wasn't willing to take Josh at his word.

Dixie approached Greg at the Valley Inn bar. She smirked that he had to give her what she wanted or she would ruin him. She informed Greg that she had proof that Josh is Erica's son. "No laws were broken," Greg insisted. Dixie was repulsed. When he refused to pony up Kate's location, Dixie added that she also knew he'd been using his own sperm to populate the world. Greg replied that if Dixie brings him down, she'll ruin her own daughter's life by taking her away from the only parents she'd ever known. Greg then informed Dixie that Kendall is in a coma and that Erica already knows Josh is her son and made a case for her keeping quiet about Josh's lineage in order to spare Josh pain. Dix seethed that Greg's manipulation was not going to work on her anymore and he had 24 hours to get back to her with Kate's location or suffer the consequences. After Dixie left, Greg summoned Tad to a meeting. "It's about your daughter," he told a confused Tad.

Erica reeled from Zach's declaration that he is Kendall's husband. Zach rushed in to see his bride after Erica informed him that she's comatose. He stroked his wife's face; she was relieved to hear his voice. Ryan pulled Erica out of the room to tell her something that she didn't want to hear, which is that despite her protests to the contrary, Kendall loves Zach and vice versa.... Zach apologized to Kendall for having failed to protect her and Spike, but Kendall knew it was up to her to pull herself out of the coma. He rejoined Erica and Ryan and got the report on how Kendall's accident had occurred. Ryan thanked Zach for getting Kendall away from Greg. Zach explained that he had separated from Kendall because she wanted to tell Erica alone that she wanted to raise her son with Ryan. Ryan's eyes widened. Zach then described the situation at Greg's ReGeneration spa and how Kendall came to realize that she loved Spike. Ryan broke down the "Kendall or Spike" conundrum for Zach.

As Julia tried to brush Kendall's hair, Kendall fantasized about emerging from her coma and was distraught to realize she actually hadn't regained consciousness. Before she came out of her fantasy, though, she watched Erica, Ryan and Zach discussing her medical options and Erica making the case for taking Spike early to improve Kendall's chances.

Zach explained to Kendall that Erica and Ryan had agreed to the C-section, which horrified Kendall; she pleaded with Zach to stop them. "Save my baby!" she cried. Erica, meanwhile, assured Ryan that they were making the right decision. Zach said that whatever happens next is up to him. "I'm Kendall's husband," he revealed.

In the hospital corridor, Babe scrambled to backpedal from having blurted out to Josh that someone had tried to kill her, but he deduced that she was referring to JR. Josh encouraged Babe to call the cops; she insisted that if JR had tried to kill her, she knew and understood why. "He just wanted to save his son," she said. Josh was unconvinced and disagreed with Babe's claim that none of this was his concern. He then warned Babe that JR still posed a tremendous threat to her. Babe returned home and played with Little Adam in his nursery. Josh showed up uninvited and told Babe that she was the closest thing he had to a friend and that he believes she lets JR take advantage of her. Babe assured Josh that she knew how to handle the situation but that she loves her husband and wouldn't take his son away from him. JR returned home during their conversation and JR glared at Josh.

In the hospital waiting room, Krystal grilled JR about the accident at the construction site. He tried to leave to check on Kendall, but Krystal said she had an offer for him. She told JR that she would do whatever it takes to get him through this difficult time. "I'm fine," he said tersely. Krystal issued a veiled threat about how she would do anything to protect her family.

In Kendall's room, a horrified, still-comatose Kendall listened in as Erica explained to Ryan that they had to save Kendall's life over the baby's. "Oh, God, no!" cried Kendall's voiceover, willing Ryan to intervene and ensure that her baby's life was privileged above hers. Ryan and Erica continued their conversation in the hallway, where Ryan pleaded with Erica to reconsider her decision. She refused and returned to her daughter's bedside. She looked up to find that Greg had arrived. She wasn't interested in his opinion, but he stated it anyway: He believed that the only choice she had was to save the baby's life. Kendall was mortified when Greg's comments — comments like, "I'm the only one who's truly qualified to decide who becomes a parent," tipped her off to the fact that he's a freakin' lunatic. Erica summoned security.... Ryan talked to one of Kendall's doctors, who explained that if they delivered the baby now, he wouldn't be able to breathe on his own, among other complications. The doc said he couldn't advise Ryan about what to do. JR entered and Ryan asked him who he'd choose in the event of a medical emergency: Babe or Little Adam. Gazing at Kendall through the door, JR said he wasn't right person to ask, but all that he knew for sure is that he would die for his son. Ryan's speechifying about not wanting his son to pay for his own mistakes hit JR in the gut. "I'm so sorry," JR said, hanging in his head in shame and walking away.... Kendall begged God to save her son as Ryan took Erica aside for a chat. With a heavy heart, he told her, "I cannot live with the idea of taking Kendall's life ... I'm going to tell the doctors to order the C-section ASAP." Just then, Zach intervened. "No, you won't!" he declared.

Trapped in the resort safe by Greg, Zach and Dixie considered their options. Zach said he had an idea about how to get out, but if it didn't work, they were dead. Zach succeeded in using nitrogen to freeze open the door.

Lily begged the New York City hooligans to respect her personal space but they continued to harass and insult her. A well-dressed man arrived on the scene and ordered the thugs to back off. Once they took off, the man volunteered further aid to Lily. She asked him to call her boyfriend, Jonathan, and deliver a message to him in code. When Lily sensed that the man might turn her in to her father, she tried to run away. The man blocked her and gave her money for a hotel room. Lily noted that he is a stranger and can't automatically be trusted; he said she was absolutely right, but that adults talk to strangers all the time. The guy said he wanted to help Lily because she reminded him of her sister. Jonathan, meanwhile, continued to search frantically for Lily. He encountered the same group of no-goodniks who'd previously harassed Lily and they beat him up. He was bleeding from the lip when he arrived at Lily's previous, now-deserted, location and cried out for her. Lily's "savior" showed up and lied to Jon that he'd never seen the girl she was looking for.

Erin rushed into Aidan's office and begged him to tell her that he knew where Jonathan was, but alas, Aidan didn't know. Aidan assured Erin that Jonathan was not a suspect in Lily's disappearance but that finding Jon was their best chance of tracking down Lily. Erin espoused the virtues of Jon and Lily's love for each other. "I love you," said Aidan. "I love you, too," she smiled.... In New York, Jon searched frantically for Lily, who was nearby in a panicked state, overwhelmed by the city's stimuli. She took refuge in a cellar and cried out for Jon. A woman approached her and tried to help but Lily bolted when the woman tried to touch her. As she left, she ran past Jon, who narrowly missed seeing her.... Aidan got word that Pete, the associate he'd assigned to keep an eye on Jon, had lost sight of him.... Jon met the woman who'd just interacted with Lily. The lady reported that Lily had just been there and had run off.... Lily was harassed by some punks in an alley. They demanded that she hand over her cash.... Erin called Jon and begged him to reveal his whereabouts. Jon refused, noting that Aidan is in cahoots with Jack.

At the Fusion construction site, Babe let the realization that JR had tried to kill her sink in. Krystal came in and sensed that Babe was upset about more than just Kendall's health. Krystal urged Babe to get real about the fact that JR poses a threat to her and Little Adam. Babe evaded Krystal's questions about the meeting JR had lured her to and took off. A suspicious Krystal remained behind and began to put two and two together about her son-in-law's actions.

JR paid a tearful visit to a comatose Kendall, who was confused by the apologies he proffered (she still thought her condition was the result of an accident). Kendall was still frantic to know where Zach was. JR told Kendall that she was right about Babe all along. As JR's monologue continued, Kendall began to understand that JR had tried to kill Babe and that she herself was comatose as a result. Unbeknownst to JR, Babe had sneaked in and overheard him confess, "Babe was supposed to walk through that door, not you." JR then saw that Babe was in the room with him, but Babe played it off as though she hadn't heard his self-incrimination. Kendall willed Babe to catch on to JR's machinations.

Erica told Joe to do whatever it took to save both Kendall and the baby. Joe informed Erica and Ryan that it was up to Erica to decide upon a course of treatment. Erica and Ryan couldn't accept that they had to choose between Kendall's life and that of her unborn son's. Erica demanded new doctors. Josh stepped in and told Erica that Kendall's doctors were the best available. Joe urged Erica to let the doctors deliver the baby early and said they had to make a decision soon. Ryan entered Kendall's room and Kendall tried to express to him that JR was a "homicidal maniac". In the waiting room, Erica gave Jack a progress report by phone. Babe and JR went into the hallway, where she told him simply, "I know, JR. I know." "What do you know, Babe?" he shot back. She said she knew that he loved her when he married her but that when he takes a hit, doubt creeps in. "You must be sleep-deprived," he said. She ordered him not to touch her and ran off. She bumped into Josh and blurted out, "He tried to kill me!"

Back in Kendall's room, Kendall was frustrated by her inability to get him to go after JR. Ryan sat down and told Kendall that he loves her; she was confused by "all the goop" and feared that he may be delivering his final farewell to her. Ryan turned his attention to Kendall's bump, addressing his son and apologizing for not having kept him safe. Krystal showed up at the hospital and asked Erica if there was anything she could do to help. Erica did not take kindly to the offer at first, but ended up telling Krystal that she had to decide who to save: Kendall or Kendall's baby. Krystal oozed sympathy. Erica noted that Krystal would choose the C-section if faced with a similar Sophie's choice. Krystal said she would try to make the choice Babe would want her to make.

Krystal nabbed JR in the hallway and proposed a chat about his "undying love" for her daughter.... Erica entered Kendall's room and informed Ryan that she'd made her decision. "We have to save Kendall," she declared.

On Greg's private island retreat with the not-so-good doc, Amanda was blown away by her luxurious new surroundings. Greg then set to work selling an impressionable Amanda on the concept of serving as a surrogate mother. Amanda asked if giving a life makes up for taking one. Greg assured Amanda that she's not responsible for righting her mother's wrongs, but that if she decided to go through with the surrogacy, he had a wonderful couple waiting to embrace the baby. Elsewhere on the island, a just-arrived Zach and Dixie approached Greg's office and Zach expertly picked the lock. They scoured the office, but had no luck finding Greg's files on Kate. Zach then uncovered a safe that held computers and more files. As they rifled through the documents, Dixie realized that all of Greg's patients had been fertilized by the same sperm donor; she and Zach were horrified at the prospect that Greg might have used his own sperm on all of his clients. Zach got his hands on a DVD, which he slipped into Greg's computer. When it played, they realized it was a video diary of sorts — specifically, Greg filmed himself on the day he stole Erica's embryo. Zach immediately recognized Erica as Greg's patient; Dixie and Zach reeled from the realization that Josh is actually Erica's son.

Greg left Amanda behind when he received an urgent call, and walked in on Dixie as she exulted to Zach that she could blackmail Greg into revealing Kate's whereabouts. Greg slammed the door shut, trapping Zach and Dixie inside the safe.

As Kendall lay comatose, Ryan left a frantic message on Zach's cell phone, then encouraged Erica to get some rest. She couldn't think of leaving her daughter's side. When Babe arrived, Erica lay into her, blaming her for the accident. Babe swore she had nothing to do with it and asked if either of them had seen JR, who was at Fusion, flashing back to the disaster he caused for Kendall. Babe took off. Ryan told Erica that he loves Kendall and needs her to recover; she gave him a hug.... Julia spoke to an unconscious Kendall. In a voiceover, Kendall wondered who was talking to her and whether Spike was okay and also why Zach wasn't by her side.... Ryan visited Kendall and begged her to wake up. She silently tried to communicate with him that she could hear him and still didn't understand what was going on.

Erica got a call on her cell phone from Jack and reported to her hubby that Kendall was comatose and may not recover. She gave her blessing to Jack to go off in search of Lily instead of joining her at the hospital. Josh approached and told Erica that he might be able to help Kendall even if Joe and the other docs say otherwise. Erica was offended by Josh's suggestion that he be allowed to experiment on Kendall, stopping just short of saying he was just like the senior Dr. Madden. Erica then replaced Ryan at Kendall's side and pleaded with her daughter to wake up. Kendall deduced from Erica's comments that she was in the hospital and, even worse, in a coma.... Joe updated Erica and Ryan on Kendall's condition, relaying the tragic news that the prognosis was not good. Joe said that Kendall's chances of survival would increase if they delivered her son early, even though the baby might not necessarily live. Joe said that Ryan and Erica had to decide how to proceed.... Kendall spoke to Spike and assured him that they were both going to pull through.

Babe met up with JR at Fusion; he was bummed that she couldn't report an improvement in Kendall's condition. When JR remarked that Kendall didn't deserve to have this happen, Babe asked who it should have happened to. "What the hell are you talking about?" JR snapped. Babe explained to JR that she'd sent Kendall to see him in the hopes that she could help him cope with Dixie's return. Babe asked JR why he had wanted her to meet him in that room specifically. He said it didn't matter anymore and stormed out. The contractor came to Fusion and insisted to Babe that he had secured the building materials. It dawned on Babe that someone deliberately sabotaged the site.

At the Fusion construction site, JR deliberately sent heavy construction materials toppling in an effort to do in Babe. He was mortified when he realized that Kendall fell victim to his plan, not Babe. He frantically tried to unearth his pregnant pal, who was unconscious. Babe, Erin and Simone arrived on the scene and tried to revive Kendall. Simone summoned 911 and the paramedics whisked Kendall away. Babe questioned JR about how the accident occurred; he was defensive and brushed past her. Babe and JR headed to the hospital.

At the hospital, Greg defended Amanda when the daughter of one of Janet's victims confronted her. Julia ushered the distraught woman away; Amanda assured Greg that she's okay.

At her office, Erica fretted to Ryan about not knowing precisely where Kendall is. Ryan dropped the bombshell that Dixie is somehow connected to Greg, but he made it clear to Erica that they can't expose Dixie's secret in their efforts to bring down Greg. Erica sniffed that Dixie needed to get over herself so that they can save Kendall, but reluctantly agreed to keep Dixie's involvement with Greg quiet. Erica reeled when Simone called to report Kendall's accident; she and Ryan rushed to the hospital, where Josh, Joe, Greg and Julia worked to save Kendall's life. Erica demanded that Greg get his hands off Kendall. Joe dismissed Greg from the OR; Greg then grumbled to himself that Kendall's accident occurred because she left his island.... Erica, Simone, Babe, Kendall, Erin and Ryan kept vigil at the hospital. Erica was shaken to see Josh treating Kendall, and an unhinged JR told himself that if Kendall dies, it will be Babe's fault. Babe, meanwhile, tried to convince herself that JR couldn't possibly have orchestrated the "accident" at the construction site.

Zach gave Dixie back the gun he found in her purse, but discouraged her from using it on Greg. Dixie told Zach that she was either going to make Greg tell her where her daughter is or she's going to kill the doc. Zach informed Dixie about Greg's private island and offered to take her there. Unaware of Kendall's health crisis, he left his wife a message telling her that he's got something important to take care of, but will see her soon.

A frustrated Tad stormed into his office and got the news from Aidan that they still haven't tracked down Lily. Tad told Aidan that he's on a mission to find out where Dixie has been these past four years. Di entered and Tad demanded that she tell him everything she knows about Dixie. Di insisted that she didn't know anything of value and noted to Tad that she wants some answers, too. Tad replied that their relationship doesn't matter when his sons and the rest of his family are suffering the way they are. He stalked off; she stayed behind to defend herself to Aidan. Aidan admitted that he thinks she's a better match for Tad than Dixie is. Tad returned and Di told him that she's not going to give him back his engagement ring. "Knock yourself out," he shrugged. Di vowed to repair their broken trust.

In his office, Greg continued to make nice with Amanda and she pledged her faith in him and his work. "Just tell me what I can do," she chirped.

Joe told Ryan, Erica, JR, Simone, Erin, Babe and Amanda that Kendall had lapsed into a coma. "We don't know if Kendall will ever recover," he sighed.
