Jason reported to Sonny and Reese that he believes Carly escaped Rose Lawn because of the St. Christopher medal he found, but Sonny reasoned the medal could belong to anyone. Corinthos insisted he'd have heard if his ex-wife was on the loose. Meanwhile, an agitated Carly told Alcazar that she thought Manny caused the train crash. She feared that Manny would hurt Morgan and Michael, but Alcazar assured her the boys are fine. He took her back to Rose Lawn before the staff detected she was missing. Alcazar said she never had to be afraid again.

A fireman approached Manny and filled him in on the rescue efforts, explosives would be used. A short time later, Sam came upon the fireman lying still on the ground and Manny crept up behind her. Manny showed her that he'd taken explosives from the fireman.

Courtney and Emily were thrilled to see Nikolas alive. Em and Jax both looked uncomfortable as Court gushed over Nik.

Alexis asked Ric if she's dying. He denied it, but she didn't believe him. Alexis asked to hold her baby and cooed to her little girl, about what a great dad she has. She cried that she wanted to see Molly grow up, but it looks like her time is running out.... Robin saw Nikolas and explained what is going on with his aunt. Nik made a beeline to Alexis and ordered her not to give up.... Ric gave the baby to Jax, Nikolas and Courtney, who promised they'd get her out safely. Later, Robin informed Ric that Alexis's situation is grim and the best thing they can hope for is that she just falls asleep. Ric returned to her side.

As they searched for a way out, Nikolas and Jax butt heads as Courtney was maternal with the baby. Nikolas told Courtney that if Ric and Alexis don't survive, he'll raise Molly..... Sam got away from Manny, who then gleefully planted explosives.

Robin and Sonny saw each other, she said that without the drug he needed, Jason's chances of survival are not good. She also told him that Alexis would not make it. Sonny went to Alexis, who asked him to take Kristina when she died. She requested that Sonny and Ric put aside their feuding and let the half sisters have a relationship.

Rescuers began to make it into the tunnel, Alexis told Rick their baby was safe, closed her eyes and let go of Ric's hand. He broke into sobs.

Jax, Courtney, Nikolas and the baby made it out of the tunnel, but Court doubled over in pain.... Sonny realized that Carly had been on the train.... Robin found her briefcase with the medication Jason needed. As they headed back to Jason, they encountered Manny, who had somehow got his hands on Sam again and was holding a gun on her. He announced that no one down there would see the light of day again.

What did the fireman give Manny?

Doctors rushed in as Lucky went into cardiac arrest. After a few tense slo-mo moments, Lucky was revived by the doctors. "Welcome back," said Luke to Lucky as he regained consciousness later. Then, Elizabeth entered, and Luke gave the newlyweds some time alone. They lamented their lame honeymoon experience. Lucky vowed to make it up to her. They cuddled. Lulu arrived at the hospital, where she saw that Lucky was okay and then she hugged her father. She thanked her estranged dad for saving her brother, but noted she wished she knew him better. Then Tracy arrived and Lulu chewed her out. Luke broke it up, and Lulu stormed off.

At the staging area, Alcazar announced a search and rescue plan. Mac said it was too risky, but Alcazar wouldn't hear of it. Justus told Lainey that he was going in. She asked him not to play hero. Maxie and Skye were worried for their men. Alcazar and Skye assured each other that their trapped loved ones were going to be okay. Jesse promised Maxie he'd come back to her. Mac queried Alcazar about why he's so concerned, but Skye covered the Carly reference, saying she'd asked him to enter the mountain to save Emily. Meanwhile, Jesse was lowered into the tunnels.

Courtney refused to leave without the cavern without Nikolas, who had been buried under rubble. The men refused to let Court help for fear of endangering the baby. Just then, they were all covered by another cave-in. WIth Nik now fully buried, Jax dragged a screaming Courtney out, as more rocks fell. Once safe, Courtney lamented leaving Nik.

Robin and Emily began Alexis's C-section, but there was a problem. With Ric's assistance, they proceeded, and Robin delivered a baby girl, but the baby was not breathing. Ric tried giving the baby CPR, but to no avail. Ric sucked amniotic fluid out of the baby's lungs, and the baby started to cry, leaving Ric overjoyed. However, Robin announced Alexis was losing blood too quickly. Jax and Court returned to the survivors and were thrilled to meet the new addition. Court offered to calm the crying baby girl, and broke down in tears when she held the girl. Jax assured Ric that Alexis will come through this. Jax and Court then had to break the news about Nik to Emily. Court gave the baby back to Ric, and then she and Jax gave Em the gory details. Em and Court both cried and embraced. Meanwhile, Alexis awoke and Ric reunited her with her baby. The three bonded and cried happy tears together. They choose the name Molly, since she's "unsinkable." They kissed and rejoiced in their new and reunited family glow. Robin returned to examine Alexis. During the exam, Robin covered when Alexis said she couldn't feel anything in her extremities, and then Robin told Ric that if help doesn't come soon, Alexis will die.

As Carly hid in the train car, she flashed back to her last goodbye with Sonny at Rose Lawn, but her movement prompted Sonny and Reese to call out, "Who's here?" Sonny wouldn't let Reese open fire. Carly created a distraction and ran off. Reese chastised Sonny, but he said his instincts told him they weren't in mortal danger from whoever it was.

A tearful Sam ordered Jason to wake up. She ran off for help, unaware of a water leak nearby. Once out of the train car, Carly stumbled upon Jason, who would have drowned in the puddle. She hugged him, dragged him to safety, and Jason woke up, shocked to see her. She then ran away. Meanwhile, Sam found Sonny and Reese and filled them in on Jason's pickle. Reese struggled with shortness of breath, but they all went off to find Jason. When they arrived at the scene, Jason told them he saw Carly.

In a tunnel, Jesse was lowered in, and located Carly.... At the antique rail car, a battered Nikolas appeared from nowhere, and Em was the first one to hug him.... Up top, Jesse brought out several survivors. Lorenzo spotted a woman cloaked in a blanket and ran over to her. He uncovered her, and it was Carly. Lorenzo embraced her, as Skye looked on.

Despite her ordeal, Alexis gave Courtney a pep talk about Jax. Emily and Ric returned with the news that Alexis needs a C-section. Alexis told Em to do whatever she had to do. "Forget about me, Emily, and save the baby," Alexis instructed. As Alexis writhed in pain, Courtney couldn't stand to watch and walked off.

At the staging area, Jax barked at Mac that he'd find another way into the tunnel. Jesse reported to the commish that there was still no word on Robin. Lainey treated patients, while Lulu barked at Luke about leaving Nik behind. Luke made no apologies for his decision. Luke checked in with the hospital — Lucky was still in surgery. Alcazar panicked when he heard the body of a deceased woman matching Carly's description was found, but he was relieved when he learned it wasn't her. Skye gave Alcazar an idea — find blueprints to the tunnel. (No one thought of this earlier?)

Nik got Elizabeth to wake up, despite the gas leak. He assured Liz that Lucky would be okay and they'd have a great life together. Even though they were trapped, she still played matchmaker for Nik and Em. The pair then heard noises from the outside. Shortly after, Jax, Mac and Jesse saved Nik and Em. Neither Jax nor Nik would leave the tunnels without rescuing Courtney, as well.

Jason pulled Robin up from the edge of the cliff. Robin told Jason that he needed to rest, but Jason balked. Sam agreed with Robin, but they gave in and decided to keep moving. Jason found Carly's St. Christopher medal in the dirt, and flashed back to when he gave the medal to Carly when she was leaving town during her postpartum depression years ago.

In the train car, Manny shot at Carly, but missed. Carly shot back, and a gun battle erupted. Carly got Manny once in the chest, then ran. In a tunnel and threatened by a snake, she shot it. Trapped nearby, Sonny and Reese heard the gunshot.

Sonny had been keeping his claustrophobia under control, and ordered Reese not to give up — they have too much to live for. Sonny kept digging and, hearing the gunshot, found access to another tunnel. During their trek, Sonny went ahead to explore, while Reese doubled over in pain and spit up blood. Sonny returned and Reese called their experience their, "last great adventure together." (Um, foreshadowing anyone?)

Jason, Sam and Robin made their way to the other train-wreck survivors. Robin assured Alexis she could do a C-section, but her HIV status worried Ric. Jason had a knife Robin could use for the surgery. "Nice to know you haven't changed," Robin quipped to her ex. Jason and Sam went off to find Sonny, Reese and Courtney. Once alone, Jason confessed to Sam that he whisked her away because of her own baby loss. He then told Sam about giving Carly the St. Christopher medal. Meanwhile, Carly realized she had lost her medal.

With everyone else preoccupied at the staging area, Luke assured Liz he'd get her to the hospital to see her husband. After an off-screen helicopter ride, Luke and Liz saw Lucky at the hospital, who was out of surgery. "Some honeymoon, huh?" Lucky groggily asked his teary wife.

Back at the Earth's core, the gang found more supplies they could use for surgery, along with some alcohol to sedate an already-medicated Alexis. It wasn't looking good.... Ric and Alexis had a warm moment before she passed out, while Robin and Emily prepped for surgery.

Back in the train car, Carly hid from Reese and Sonny. Reese found a gun and recognized that it was no cop's weapon. Hmmm. Carly flashed back to her good-bye with Sonny at Rose Lawn and then bumped into some wreckage. Sonny demanded to know who else was on the train with them.

Jax and Nikolas found Courtney passed out in a tunnel. She came to, and gave them an update on the survivors. Court surmised she must have fallen and hit her head. Jax barked at her, then apologized. Then, another cave-in left Nikolas buried in rubble, and despite Jax's protests, Courtney refused to leave him.

Making their way along a ledge, Jason suffered another headache and fell down a shaft. He was unconscious, so Sam climbed down to get to him.... At the staging area, examining the blueprints, Alcazar came up with an idea.... After the warm reunion at the hospital, Lucky went into cardiac arrest.... With Alexis knocked out, Robin reassured a worried Ric, and the operation began.

The train wreck survivors fretted about Alexis having a C-section, which appeared not only impossible to perform under the conditions, but which the mother-to-be refused anyway. Ric apologized to Alexis for his selfishness, and she forgave him, and Emily found some meds to slow down Alexis's contractions. For the safety of the child, Alexis took the meds. The pills seemed to take effect, as Alexis and the contractions eventually quelled.

Liz gave Lucky CPR and revived him. Lucky screamed in pain as the gang improvised and put him on a makeshift stretcher. Nik volunteered to help Luke carry Lucky to safety. Nik bid his aunt Alexis farewell and assured her they'd be back for her and her baby. He then bid goodbye to both the women in his life.

As Manny taunted Carly, she took a swing at him, but missed. Manny wasn't so lucky the next time, but he still managed to overpower her. Manny threatened to "take" Carly before he killed her. They struggled, as creepy Manny kissed her neck. An explosion gave Carly the chance to push Manny off her. She hit him with a rock and escaped.

Unable to free Robin from the wreckage, she told Jason to get out and save Sam. They refused, and managed to free Robin. Once outside the car, Robin said they can't leave without her research for his treatment. Sam told Jason that Manny was on the train with them, but that didn't dissuade him from heading back into the flaming train car. Once inside, Jason suffered another headache.Sam and Robin were stunned to see the car explode, but Jason leapt out just in time. Or had he? The women found him unconscious. Jason came to, but didn't find Robin's briefcase. He then informed the women what Tony told him before leaving the hospital — that the next seizure could kill him. As Robin lamented she didn't take a plane, Sam was more preoccupied with the fact that today would have been her baby girl's birthday. Not wanting to sulk, however, Sam suggested that rather than wait for rescue, they should look for a way out.

Jax and Alcazar argued as they attempted to dig out the escape tunnel. Jax remained skeptical of Alcazar's true motives. However, the men put aside their difference to work together. At the staging area, Tracy demanded to know what secret Skye was hiding. The women bickered until Lulu showed up and barked that Luke would never love either of them. Jesse reported to a dejected Mac that no one so far had seen Robin. Lainey tried to keep it together as another survivor didn't make it through triage. As Tracy and Lulu bickered, Maxie and Dillon showed up at Skye's request to diffuse the fireworks.

On their journey to the surface, Liz, Luke, Nik and Lucky reached a dead end. Luke was furious, but Lucky offered his dad some sage advice about finding the weakest link. After doing some digging, the gang heard voices, and a rescue team broke through. "I never thought I'd be happy to see your face," Luke told Alcazar. Just then, Lucky took a turn for the worse. The gang got Lucky through an escape passage, which collapsed on Luke and separated Liz and Nik from the rescuers. Frantic about not knowing what happened to Lucky, Liz passed out. As Dillon swept Lulu out of Tracy's firing range, Luke, Jax and Alcazar made it to the surface with Lucky.

Skye assured Lorenzo she kept mum about Carly. Georgie apologized for Lulu's bratty behavior, but he said he totally understood her. Lulu wanted to accompany Lucky to the hospital, but Lainey shot her down. Lulu accused Luke of leaving Nik to die. Nik realized there was a gas leak, just as he collapsed trying to carry Liz to fresh air. Alexis advised Courtney to avoid mass transit when her due date approaches. Emily pulled Ric aside and told her a C-section was still needed, and Alexis and the baby are at risk. Walking on a ledge to safety, the ground fell out from underneath Robin, as Jason attempted to pull her to safety. A gun-toting Manny pursed Carly. She found a gun in the wreckage and readied for his arrival. Stand off!

As workers outside of the tunnel struggled to get to the train crash survivors, those stuck inside fought for their lives. A badly injured Lucky said a fond farewell to his new bride Elizabeth and stopped breathing. Alexis was horrified that she'd gone into labor, but Ric stuck by her side. Reese nursed her mystery wound, just as a cave-in separated Lucky and Liz from the others. Liz awoke covered in dirt, and was saw a pile of rubble separating her from her betrothed. Nikolas and Emily leapt into action to dig them out, followed by the other captives. Liz frantically dug through debris to locate her ailing husband. She uncovered a hand, but cried when she found no pulse. Luke arrived down a ventilation shaft in the nick of time and was able to free his injured son. Liz checked him again and was able to pick up a pulse.

Outside, Skye and Lorenzo fretted for the survivors, just Tracy showed up and picked a fight with Skye. Jax was able to climb out of the tunnel and give the emergency workers info on his trapped companions. Skye was suspicious about why Alcazar was so concerned about the wreck, until she deduced that Carly must be aboard. Jesse assured a blustery and frantic Mac that Robin would be okay.

Carly awoke and managed to keep hidden from Robin. Jason made his way to their train car, but didn't recognize Carly in the dark. With his instruction, she was able to extinguish live wires before they could ignite a blaze. By the time he climbed inside the car, however, Carly was gone, but Jason found — and recognized — Robin. Robin was still conscious, and berated him for leaving the hospital in his condition. She then sent him off to search for Sam, whom he later found buried alive.

Skye called Alcazar's bluff, and he copped that he believed his estranged wife was aboard. Skye flipped, thinking Carly may have caused the crash. Lorenzo, however, refused to pass judgement.

Lainey vented her frustrations to Justus after Jax refused to stay topside and went back into the tunnel with Alcazar. Justus was able to calm her, and after a warm moment, they got back to work helping others. That tunnel, however, proved to be a dead end after the last cave in and Jax and Alcazar vowed to find another way in. Skye wished Alcazar luck, as Tracy watched.

As Ric assisted Emily in searching for supplies, Reese agreed to keep Alexis company. "The irony," Alexis not-so-subtly noted. Ric and Em found some blankets and a first aid kit, but Courtney started cramping, and Nik summoned Em to her side. Court was told to rest and not exert herself. Lucky regained consciousness, just as Nik made it through the debris. Sonny vowed that if Luke could find a way in, he'd find a way out. Reese vowed to stay with him. In the tunnel, Max nearly caught up with a fleeing Carly, just as Manny showed up and shot Max. Carly pulled a lead pipe on Manny.

Lulu dragged Georgie to the staging area under false pretenses, but Jesse had no new information for them. Georgie refused to let Lulu get in the way, but Lulu stuck to her resolve to do something of importance. At the hospital, Alan reassured Monica that Jason and Emily would be okay. As Jason held Sam and begged her to come to, she did. Sam was able to stand up, and together they went back for Robin.

Despite her injury, Reese refused to leave Sonny's side due to his claustrophobia. But, another cav- in caused the pair to become trapped.... Luke refused to let Lucky give up and pleaded with him to live. Lucky stopped breathing.... Alexis wondered why she and Ric can only get along in a crisis. Without admitting as much, the pair agreed to work on salvaging their marriage. Emily then broke the bad news to Alexis and Ric that she was going to need a C-section to deliver.
