As Courtney and Nikolas cuddled, Dr. Lee entered with the news that Court's fever broke and they could proceed with the c-section, but it was still risky. Jax and Nik worried that things didn't look good for the baby. As the men fretted, Courtney was all smiles and couldn't wait to have her child. She asked Jax to promise her that they'd raise their baby in peace. He agreed. Then, Emily overheard Nik and Courtney planning their future together and walked away. Before her surgery, Mike visited with Courtney. They shared a warm moment, and he claimed first dibs on babysitting duties. "Let's have this baby," she declared as they wheeled her into the O.R. Nik and Mike watched through the window and spoke about their fears. Nik started feeling faint, and Mike went in search of a doctor. In the O.R., Jax was a pillar of support for Court. As the baby went into crisis, Nikolas fainted. Noah gave Nik a dose of the serum, and Court delivered the baby — a boy — but it wasn't crying.
On the island, Carly went forward and injected herself, still not sure if the dose was the virus or the cure. As they waited, Carly found a beer, opened it and drank it. Meanwhile, Lucky called Liz, who was relieved to hear that her hubby was okay. He told her about the antidote, but they got cut off. Lucky then joined his cousin and Jason, just as Carly stood up and announced that her fever broke. It was the antidote! Lucky put the remaining antidote in his pocket. Just then, Crylium goons busted in and shot up the place. Jason played possum, then shot all the bad guys. The bad news was, the antidote vial broke when Lucky dove for cover. Jason pulled a gun on one of the thugs who'd survived and demanded to know where more antidote was. "Gone," stuttered the thug. Carly found a shipping bill stating that antidote had been sent to.... somewhere in Port Charles. Meanwhile, a mysterious package arrived at the Quartermaines but before Alice could pick it up, someone shot her with a blow dart and took the box.
Emily collapsed just after Sonny and the kids left the hospital safely. Luckily, Elizabeth and Emily found her moments later. Sonny returned home with the kids in tow, then got a call from Liz who informed him that Emily collapsed. Em woke up, and Monica returned with the test results. Emily did not have the virus — she was just exhausted. Patrick wanted to send her home, but Emily fought it. Liz and Monica told Em that she was not going to win this one, and Em conceded to leave. Sonny returned to the hospital and found Emily in the parking lot. They hugged. Sonny said that it was time for her to be taken of. Back at Sonny's Michael was thrilled to see Emily. Once alone, Em told Sonny that she feels good again about wanting to be a doctor. They kissed.
Lucas visited with Maxie, who said she owed her life to Nikolas. She offered her condolences about Tony. He told her that compared to what was going on, being gay didn't seem like that big of a deal. Maxie told Lucas that she was afraid she was going to die. She attributed her past recklessness to her feeling of living on borrowed time, and stated she didn't want to die in the hospital. She made Lucas promise that he would see to it that that didn't happen. He agreed.
As another body was wheeled past her, Tracy went to check on Luke. To Lulu's amusement, Luke shared Tracy's vow to be a nicer person in exchange for sparing Dillon. Tracy smiled, and Luke and Lulu went, "Ewww." On Lulu's behalf, Luke asked Noah to do something to help Nikolas. Meanwhile, Dillon recovered from his bout with the virus, and he and Georgie were thrilled to be husband and wife and were ready to start their life together. Though due for release, Noah entered and asked Dillon to stay. Dillon attributed his ability to survive to having Georgie as a reason to live. However, Noah wondered if Dillon might have the same antibodies in his blood that Luke did. Dillon agreed to be tested, and reassured Georgie that he'd be okay. They hugged. During the procedure, Dillon's blood pressure dropped. Tracy entered and accused Georgie of being behind this. Luke was right behind her and reassured Dillon that he'd be okay. Dillon pulled through, and Lulu expressed her gratitude. Then, Liz told the Spencers that Lucky was going to be okay, too, much to their elation. Later, Tracy visited Dillon in his room and told him that she was proud of him. Dillon and Georgie were stunned. She told them about turning over a new leaf, but then asked him to wipe "this ridiculous marriage off the books." When they refused, Tracy pulled out the big guns. She said that Mac's consent form held no weight since he wasn't her primary guardian. Just then, Georgie cell phone rang — it was Felicia. Tracy walked out thinking her work there was done, and Luke chastised her for her short-lived goodness.

Em told Nikolas that Maxie was doing fine, though she's still feverish. Lulu visited her oldest brother, who said he was happy to see her well. She relayed how proud she was of him for saving Maxie. He asked Lulu to find him a wheelchair.
Alexis told Jesse that Maxie was doing okay. He was relieved and asked Alexis to thank Nik for saving her life. Alexis showed Ric some new pictures of the girls, as his condition seemed to deteriorate. Alexis looked at the dark side of what if's, but Ric reassured her that everything would be okay. Alexis then climbed into bed with her hubby.
Patrick informed Sonny that he made a full recovery and told him that he was being sent home. Patients who make a full recovery are no longer susceptible to the virus, explained Dr. Drake. Sonny asked Em to come home with him, but she wouldn't leave the sick patients. Sonny told Em he couldn't have made it without her. Sonny dressed and asked Monica about Sam's condition. She's stable, Monica replied. She then asked Sonny to talk to Sam and allay her fears about Jason. During their talk, Sam told Sonny she's concerned about Danny, then feared that Jason might be sick or even dead. Sonny visited Ric to check up on him. Sonny said their mother was watching over both of them, and Alexis asked Sonny to take Molly and Kristina home, along with Michael.
Dr. Lee let Courtney know that surgery was still not an option due to her fever. The doc said it appears the baby is fine, but she wouldn't risk the C-section yet. Court refused to even consider the idea of her baby dying. Sonny stopped down to see Courtney, and Jax gave him an update. Sonny noted that Jax risked getting infected to stay with Court. Sonny sat with his sis, and told her that he understands what she wants to do. "You're in good hands," he assured her as he left. Lulu brought Nikolas to see Courtney, and left to give them time alone. Jax gave Nik the update. Court was delighted to see her lover. They talked about the baby dilemma, and Jax. She insisted her fever was going down and today was the day her baby was to be born. Dr. Lee came in and validated Court's claim. It's full steam ahead!
Michael was overjoyed to see Sonny well and learn they were going home, as Alexis went back to her room and told Sam that she sent the girls home with Sonny. Sam asked Alexis what she was referring to in the chapel last night, when Ric was hallucinating that Alexis was his mother. (You remember, when Alexis said something about not having a choice to leave her child. Hmmm.) Sam retreated, saying she needed to mind her own business. Alexis explained that she was pretending to be Adela for her fever-addled husband's sake.
Elizabeth let Luke know she heard from Lucky and the news wasn't good: she thought he was calling to say good-bye. Luke vowed to leave the hospital and find his son, then told Liz not to count her husband out yet. Luke went to Scorpio with information about Lucky and mentioned the name Crylium Industries. Scorpio was shocked, but told Luke that even though they used to supply the military, he wasn't aware of their involvement. Scorpio was furious no one came to him sooner about this, but Luke barked back that he needs to do whatever it takes to get Lucky home. Lulu showed up and begged Luke to help Nikolas. Luke was determined to help Lucky, instead. Eventually, he surrendered to her request and gave more blood. Noah warned Luke that it was too soon. "You're beginning to sound like a broken record," Luke quipped back. Noah shut the transfusion down. Luke was crestfallen, but Lulu thanked her dad for trying. Meanwhile, Liz went to the chapel and prayed for Lucky's safety. "He's a gift from you," she said to the heavens. "Please don't take him away from me." Patrick watched silently from the hall.
In the jungle, Jason discovered a body bag and opened it. It wasn't Lucky. Whew! Jason took the stiff's security pass before leaving the corpse behind. Thugs came along and noticed the body had been disturbed. As she and Jason hid, Carly collapsed. She recovered, and the pair found a hidden fortress. The security pass gave them access, and inside the complex, Jason and Carly found Lucky, who told them he was given a new antidote and it worked. Jason found a vial, but they couldn't tell if it was the virus or the cure. Carly volunteered to be injected with it. Jason refused to let her, but she pulled out the needle and was about to inject herself.
On their way out of the hospital, Scorpio stopped Sonny and ordered him not to leave. Uh-oh! Robin and Emily showed up and assured the old guard that the new guard was immune. When Scorpio chastised his daughter for hanging with the wrong crowd, she shot back, "Sonny would never have treated me the way you did." Downstairs, Emily watched Sonny, Max and the kids leave, then collapsed.

As Dillon's conditioned worsened, Tracy sat in the hospital chapel and prayed for her son to be spared. A feverish Dillon dreamed of an imaginary wedding to Georgie — the wedding of their dreams with the gazebo resembling a winter wonderland, with he in a dashing tux and she in the picture-perfect wedding gown. They recited their romantic vows, shared cake and champagne and toasted to their bright futures. Then, like Fred and Ginger, they shared their first dance as man and wife. Georgie asked Dillon for just one more thing: "I need you to live." Just then, Dillon gasped and woke up, much to Georgie's relief. When they spoke, they realized they had both had the same exact dream. Back in the chapel, Tracy struck a deal with God saying that if Dillon is saved, she promised to be a good person from now on. Luke startled her when he eavesdropped and moaned, "I heard that." Luke scoffed at Tracy's ability to change, but said that, if anything, she'd better thank Georgie for giving Dillon a reason to live. Tracy then went to check on Dillon and saw he was awake. "Uh-oh," taunted Luke, "Looks like you got your miracle."
Courtney's situation looked grim: if she doesn't have a C-section, the baby will undoubtedly catch the virus and die. If she does undergo the procedure in her weakened state, it could kill her. Jax begged Courtney not to risk her own life to save the baby's, but as she reminded him, Court was considered infertile. This was a miracle baby, and there was no way she wouldn't do whatever she could to see that the baby lives. At her urging, Jax vowed to support Court in her quest. Dr. Lee prepped Court for surgery, but later announced that the operation was no longer a possibility. Court's fever was too high. She suggested waiting to see if her fever comes down.
Stuck in the hospital elevator, Nikolas yelled for help as Maxie suffered a heart attack. Mac and Emily managed to open the doors a tad, but the car was stuck between floors. CPR wasn't helping. As Mac tried desperately to free them, Emily passed Nikolas a defibrillator and instructed him on how to use it to revive Maxie. Nik tried, but Maxie was unresponsive. Nik then started CPR again. Nothing. Mac grew frantic. Emily passed along a syringe and instructed him to insert it directly into the heart. Nik was reluctant, but followed Em's instructions. It worked — her pulse returned. Manny showed up and told Mac if he could borrow a pocketknife, he could get the elevator started again. Mac was hesitant, but did what he had to to save Maxie. Manny's plan worked, and Maxie and Nik were freed. Mac shook the crook's hand, then he thanked Nik and Em for saving his daughter's life. Monica examined Maxie and stated that the fever's straining her heart.
At the Crylium Lab, the antidote wasn't helping and Lucky's health continued to deteriorate. Preparing for the inevitable, Spencer was able to make one last phone call to Elizabeth. Liz was relived to hear her husband's voice but grew panic-stricken about his failing health. After telling Liz he loved her, they got cut off. Elizabeth was heartbroken and riddled with fear as Patrick tried to allay her worries. However, she refused his offer to take a break and instead devoted herself to helping the sick, refusing to believe the worst would happen.
Elsewhere on the island, Jason and Carly were hot on Lucky's trail. Carly spotted the chimp and realized they were close. The chimp remembered Jason, and Carly managed to communicate with the primate, who led the intrepid duo into the jungle. Just then, they heard a noise. Could it be Lucky?

Jason did the honorable thing and rebuffed feverish Carly's advances. Later, her fever broke and Carly was embarrassed when Jason recounted back her words and actions. She did say how grateful she was to have him in her life. "You ever wonder what might have happened?" she asked. He ducked out of a response as Carly recounted her wayward past actions and pondered how their lives could've been different. Jason then said he's glad that while he loves her, he's not in love with her, because that's why they're still together as friends, since she can't break his heart. Later, with the helicopter on its way, Carly was too weak to travel, but Jason refused to leave her behind and pleaded with Carly to find her inner strength.
Back in Port Charles, Patrick got a feverish Ric back to bed, but not before Lansing chastised the doc for his lousy bedside manner.... In Sonny's room, Sam started to lose it with worry over Jason, despite assurances from Sonny and Emily.
In the chapel, under Tracy's skeptical eye, Dillon and Georgie exchanged heartfelt vows and were wed, though they declined a kiss at this point because he's still contagious. After Mac wheeled Dillon back to his room, Maxie suffered shortness of breath and had to sit down, causing Lucas to grow concerned about her heart. He ordered her to consult with Monica to see if she was at risk.
Downstairs, Dr. Lee gave Jax the news that Courtney and the baby were in dire straits. Meanwhile, Nikolas proposed to Courtney again, stating he'd bought an engagement ring that he left on his dresser. "I will love you for as long as I live," replied Court. "Yes. Yes, Nikolas, I will marry you." Jax overheard, and Nik said he'd never come between Jax and his child. As Monica spoke with Courtney, Maxie arrived. Monica asked Max to help Nikolas get back to bed. Maxie agreed, without speaking up about her heart concerns. Then, Maxie got anxious when their elevator stalled. She called for help, but no one was available. Meanwhile, Jax explained to Courtney that the C-section was needed to keep the baby safe from infection, but it would endanger her own life. Court demanded the procedure immediately, but Jax said no.
Later in the chapel, Sam found Ric, who was flustered that he didn't know how to pray. He mentioned that his parents never took him to church or to Sunday school, and talked about the mother he never knew and how she chose Sonny over him. Sam returned to her room, where Alexis woke up and after hearing of Ric's condition, Alexis panicked and wanted to see her husband. Sam wheeled a weak Alexis to the chapel, where Ric was hallucinating and he chastised Alexis, believing she was his errant mother. Strangely, when Ric shouted at her, "How could you give up your own child!" Alexis replied, "I didn't have a choice."
In Dillon's room, the newlyweds had a private moment and despite his illness, Dillon asked Georgie for one dance. "I love you, wife," he said before collapsing. Shortly thereafter, Tracy and Georgie squabbled over Dillon's declining health, then Georgie entered his room and begged Dillon to come back to her. She climbed into bed with Dillon and cried herself to sleep with worry.
Patrick examined Sonny, who noted the serum was working on Corinthos. Later, Emily indulged Sonny with a trip to the roof, where he assured her that he wasn't going back to Carly and apologized for pushing her away.
In the elevator, Maxie and Nikolas had a heart-to-heart about matters of the heart. Afterward, Maxie collapsed and Nikolas screamed for help.

At the Maarkam Islands lab, that evil Dr. Cassius reported that the antidote wasn't working on Lucky and he needed a new test subject for the virus, then leered at Carly. Jason showed up in a blaze of bullets and took out the doc and some goons, but not before the baddie informed them he stuck Carly with the virus, after all. More goons showed up and the gun battle continued. The goons got Lucky and escaped, then Carly collapsed. Jason took Carly back to the hotel, where a doctor examined her. Jason then called Sam to check in on her, but they got cut off before finishing their conversation. Just then, the doctor was about to give Carly an injection and Jason pulled a gun on him. Jason's hunch was right. The doc was a Crylium toadie, who then spilled where they took Lucky. Jason called the authorities, filled in the local law about Crylium, and they agreed to help the Americans find Lucky. Once alone, Carly began hallucinating and tried to seduce Jason.
At Dillon's bedside, the feverish young man declared his love for Georgie to Tracy, who criticized his melodramatic gesture to marry his love and then die. Georgie eavesdropped as Dillon gave Tracy her own words back to her about the fragility of life. Though not happy about it, Tracy gave him her blessing and said if Mac consents, Tracy will not stand in their way. Georgie shed a tear and thanked Tracy. Maxie showed up and said that she would take care of Mac. At the nurses station, Maxie slipped Mac a consent form for Georgie's marriage. He freaked, fearing Georgie was pregnant. "I'm not pregnant, " Georgie assured, "I'm in love." Mac wasn't happy about it, but seeing how much this meant to Georgie, he signed the consent form. Meanwhile, Tracy and Dillon shared a heartwarming talk, where she told Dillon that she'd accept him marrying so far below his station that he would get the bends. Then, getting emotional, she left the room. Georgie entered and gave Dillon the good news about Mac. He asked her to be sure of her decision. She assured him that she was. Tracy, Lucas, Maxie and eventually Mac joined Georgie and Dillon in the chapel. The priest began the wedding ceremony.
Patrick fretted with Emily over who to give the antidote to so he flipped a coin, but Nik interjected and said to give the serum to Sonny. Nik claimed that since Sonny has kids, the antidote should go to him. "You don't have to do this," stated Emily as she shed a tear. "It's the only logical choice, Em," noted Nik. Weepy Emily went to Sonny's bedside and informed him of Tony's demise. She then told him that he was next in line for the antidote, but Sonny was suspicious. He realized Nik must have passed it up, then he refused. Nikolas told Sonny to take the meds for his kid's sake. Patrick entered and said the virus was spreading in Sonny's blood stream. Against Sonny's wishes, Patrick gave him the antidote. Later, Nik told Sonny he knows Emily loves him. Just then, Liz entered with the news about Courtney. Nik went off to see her.
Moments before, Jax showed up at the hospital, just as Courtney was unloaded from an ambulance. Liz was shocked to see Court admitted and realized she should inform Nikolas of this surprising development. Jax explained for the audience's benefit how Courtney had escaped Helena's clutches and she continued about how she snuck back into town and was staying in hiding since she needed more time to think. Just then, Nik hovered nearby. Jax gave them a moment alone. Nik was shocked to hear of Helena's involvement, then Court rendered her decision — "It's you. It's always been you." Nik kissed her as Jax watched helplessly. Dr. Lee then approached and told Jax that Courtney needs a C-section, and that she and the baby are both at risk.
Monica arranged a visit between Sam and her brother, Danny. After their nice sibling moment, Sam tried to calm Danny's fears. Afterward, Sam went to Sonny's room, where he and Emily were pledging their devotion to each other. Sam broke the news that Jason called — and Carly's sick.
