Ethan persisted to get Theresa to come clean with him, but to no avail. Jared entered the scene and asked Theresa what she was doing down on the wharf. Theresa explained how they were pursuing a wild goose chase of mysterious text messages with Whitney and discovered Chad and Vincent having sex. Jared admitted he knew what Chad was up to and warned him to stop. Ethan was annoyed and felt that Jared should have done more. Jared said it wasn't his place to butt into someone else's relationship. Theresa reluctantly left with Jared. Alone with Jared, Theresa flashed back to the blackmailer's threats and quietly lamented her predicament while keeping up appearances. Meanwhile, Ivy caught up with a tearful and dejected Ethan. Ivy insisted that he's too good for Theresa. "She's not worth it," Ivy whispered to her son before leaving him. Ethan kept getting the feeling that someone was watching him on the wharf.

Tabitha warned Kay against playing with fire when it came to attempting to use magic to get out of her dilemma with Fox and Miguel. Kay insisted that she could use her powers to heal Fox so she could go back to Miguel, but Tabitha said she's not yet capable of controlling her power and things would end in disaster. Upstairs, Fox grew impatient having his "treatment," so his nurse helped him pass the time while he finished his placebo IV. Downstairs, Miguel asked to speak with Kay, and asked her again why she left him to go back with Fox. Kay reluctantly lied through her teeth to keep up her end of the bargain with Julian. Up in the attic, Tabitha was downing a few martimmies while reviewing spellbooks. Once she fell asleep Kay snuck upstairs and tried to yank the book out of her hands. Kay finally managed to yank the book free and began flipping through it, looking for the right spell to help her. Kay began chanting one the spells when the house began to shake violently.

At the hospital, Sheridan and Fancy freaked as the computer screen went blank before the blackmailer's picture could download. In jail, Luis yelled for the guards to summon Sam. Luis told Sam his worries that Fancy might be in danger if the blackmailer realizes she's hot on his/her trail. Later, Luis spoke with Sheridan and made her promise to keep an eye on Fancy for him. Sheridan explained how the computer crashed. Luis surmised that the blackmailer must have been a patient at Harmony Hospital at one time, and told her and Fancy to cross-reference the record number on the computer screen with the hospital medical records to see which doctor ordered the blood test. Meanwhile Sam and his men followed the blackmailer into the air ducts, where he/she cut the cord to the hospital computer. The blackmailer dropped into Dr. Smith's office. Dr. Smith recognized the blackmailer as one of his patients when he/she took his/her mask off. The doctor commented how he/she stopped seeing him abruptly after beginning "to address some issues." At the blackmailer's behest, the doctor agreed to drop everything and pull his/her file to go over his/her test results.

Kay doted on Fox and asked him how he's feeling. She tried to lift Fox's spirits by pointing out he's now receiving his treatments at home with her and not in the hospital. Fox thanked Kay for being such an angel about all of his health problems. He played on Kay's sympathies and said it might have been better if he just turned down treatment and died so she wouldn't be stuck "with an invalid of a husband." When the nurse began his treatment, Kay excused herself to talk to Tabitha and spied Miguel through the window, sitting outside. Tabitha stopped Kay before she could join Miguel and admonished her for almost breaking her pact with Julian. Kay aspired to hone her magic powers and use them to take care of her problems, but Tabitha cautioned her against getting herself in danger with the forces of the dark side. Kay had a fantasy in which she decided to go back to Miguel, but a demon Julian warned her not to break their promise. In the fantasy Kay "healed" Fox so that he'd be well enough to find someone else to love. She then banished Julian so she'd be free to return to Miguel. Tabitha brought Kay out of her daydreaming and warned her not to use any magic on Fox or Miguel.

At the Blue Note, Julian recovered from Ivy's slap. An angry Ivy accused him of being "the same self-serving pig you've always been," and insisted he's never changed. She then turned her scorn on Theresa and asked her why she so willingly gave up her Crane power without a fight to "a man she professes to despise." Theresa tried to stonewall and said she just wants to spend more time with her children. Ivy persisted and asked if Julian was blackmailing Theresa. Ivy told Ethan to look at Theresa and notice how scared she is. Ethan asked her point blank if it was true. Theresa denied Ivy's accusations, but Ivy remained undaunted and warned Ethan not to let Theresa take any more from her than she already has. Ethan turned to Theresa and insisted that she tell him her secret. Theresa grabbed her bag and ran away, and Ethan ran after her. He caught up with her on the docks and asked her what's wrong. A tearful Theresa insisted she couldn't. Meanwhile, Julian asked Ivy for another chance. "I'm not the fool I once was," huffed Ivy. Julian told Ivy he loves her, but Ivy was having none of it. She told him to go find someone who doesn't know him. "Or better yet, go find Theresa. You're both rats from the same sewer."

At Harmony Hospital, Sam and Sheridan tended to Fancy after the he/shemale, dressed as a janitor, tried to strangle her. Sam assured her that the hospital was on lockdown so the attacker can't escape. Fancy wondered why Sam was looking all over the hospital for her when Sheridan knew Fancy's whereabouts the entire time. When Sam and Fancy realized that Sheridan told Sam she had no idea where Fancy could be, Fancy slapped Sheridan. "You were going to let that freak monster kill me!" she ranted. Sam broke up their altercation before they got into a full-blown fight. Sam demanded answers from Sheridan. Sheridan admitted she knew Fancy's whereabouts all along. Sam got a call from his officers saying the blackmailer had been cornered, so Sam left to join his men, who holed up him/her in a supply closet. On the count of three, Sam's men stormed the closet to apprehend him/her. Meanwhile, Fancy and Sheridan returned to the hospital computer where the blackmailer's photo was being downloaded. Just as the download was about to complete the screen went blank.

Whitney and Theresa arrived at the Blue Note to find Chad mauling some woman on the dance floor. Ethan explained that Chad was trying to prove his heterosexuality, but no one was buying his "desperate" act — not even when Chad pulled a second woman into his slow dance. Fed up, Whitney interrupted and told the women to get lost. She slapped Chad and scoffed, "To prove that you're not gay, you go make yourself a skank sandwich?" After hurling more insults, she stormed off. He chased her down and threw their vows at her, but she shot, "When I married you, I wanted a real man, not a husband who wanted to be with another man." She even said that since he was her first lover, "I don't know if you gave me what a real man is supposed to give a woman who he says he loves." He was frustrated that no one believed he wasn't gay, but finally walked away.

At a table, Ivy and Julian toasted each other. She flashed back to their hard — and campier — times. There were a lot of murder attempts in there, so it was no surprise they hoped it would go better "this time." Then Ivy suggested they move away from "the shadow of Crane Industries," and started planning a trip. When Ethan saw them dancing, he cut in and asked his mom what was going on. She admitted they were "reconnecting" as a couple and took his disbelief in stride, noting she was no less supportive of his relationship with Theresa. Julian and Theresa, meanwhile, laughed their own round of insults at each other and then Julian seriously suggested she tell Ethan the truth about Little E. When he told her Ivy wanted to leave the Crane name behind, she told him to tell the truth — or even go through it for Ivy. "If you insist on surrounding yourself with power instead of people who love you, you are going to end up alone," she warned, but it was too late. Ethan was crushing his mother with news of Julian's new power and responsibility at Crane, while Theresa imagined all Julian could do if he sold Crane and did good with the money. Unfortunately, Julian dreamed about taking Crane "from a global empire to an interplanetary colossus," and Ivy overheard. She slapped him and stormed off, once again letting Ethan comment to Theresa about how all secrets come out.

Paloma's answer to Noah's proposal was put off by business when results came in on the fingerprint Paloma lifted from one of the dead johns. They were relieved to see the prints weren't Jessica's, but Spike's. It wasn't strong enough to hold up in court, but it was "compelling evidence that whatever happened, Jessica didn't act alone." Meanwhile, Jessica caught Simone up on all the happenings and worried she'd be caught as the serial killer, but then Simone wondered if Spike was the real killer. After making some wishful premonitions, Simone went to get them burgers, leaving Jessica alone when Spike came knocking. She stood up for herself, but Spike remained cocky as ever about her guilt. He stopped laughing when she said she was going to tell her father he was guilty. He forced a liquid down her throat and she passed out.

In Eve's office, the blackmailer tried to strangle Fancy, who was closing in on his/her DNA match. Sam was nearby, but when he told Sheridan that Luis feared for Fancy's safety, Sheridan played dumb instead of telling him where to find her niece. Then she fantasized about finding Fancy "too late", while Sam called backup to the hospital. When he suggested they split up and find Fancy, Sheridan heard the scuffle in Eve's office but again misdirected Sam. "I'm so sorry, Fancy. Please forgive me, but Luis belongs with me, not you. You were just — well, bad timing. Your time is up," she shrugged, but then Fancy and the blackmailer burst out of the room. Sheridan was forced to help and call for Sam. The blackmailer ran off, so Sam had the hospital sealed while Sheridan pretended to care about an unconscious Fancy.

Theresa came by Whitney's to offer her support, reminding her, "You always have me." Whitney said Theresa was the one person she could always trust. (That's when you know you're in trouble!). Then she wondered if Chad turned to Vincent because she wasn't good enough in bed, but Theresa insisted Chad did it because he was gay, even if he was denying it. She promised her friend would find love again, but Whitney was swearing off men for good. "You should be on the same page with me here about love, right? All it's done is tear your life apart," Whitney challenged, but Theresa begged her not to give up on love. Then she suggested they go out and celebrate moving on. Whitney didn't feel like celebrating, but agreed to hit the Blue Note to hear some music and forget about Chad. Too bad that's where Chad was! Over drinks with Ethan, Chad was offering any excuse for his behavior except being gay or bisexual. Ethan was confused, especially after he caught Chad "cruising" a guy at the other end of the bar. Chad denied it, saying he thought he'd recognized the hunk as a co-worker from West Hollywood. Intent on proving he's straight, Chad picked up on the first woman who walked by. He put on the hard sell and she fell into his manly Crane arms. When Whitney and Theresa arrived, Whitney immediately started to loosen up. Then Theresa saw the display Chad was making on the dance floor.

Luis woke from a nightmare in which the blackmailer had killed Fancy, so he called for the guard in hopes of making a call. Instead the guard decided to move him back to solitary, but Luis threw the rules in his face and stopped him. When the angry guard left, Luis noticed he'd dropped his matches. He lit one and set off the fire alarm. The guard called it a false alarm, but not before Sam showed up. Luis caught him up on Fancy's investigation and his fears for her, while Fancy worked the DNA at the hospital. Sheridan caught her niece breaking into Doc Russell's office. Cue the bickering, amid which Fancy filled Sheridan in on trying to find a DNA match. Sheridan insisted they team up. Fancy worried the blackmailer had followed Sheridan — and rightly so. He/She was dressed as a janitor and plotting to stop them "dead". When the girls cheered over closing in on a match to the DNA, he/she growled that they'd get "matching coffins". It was taking awhile for the program to find the results, so Sheridan went to get coffee. That left Fancy alone for the blackmailer to make his/her move.

Noah told Paloma his grandmother's engagement ring was "flawless, just like you." They awed over the family heirloom and he talked about his grandparents' amazing love. "My father gave me this ring now…because he believes I have found the right woman," he declared and got down on one knee. Paloma was touched by his proposal, but worried they were being selfish. "There's so much trouble around now that maybe we should focus on helping our loved ones, not thinking about marriage." Noah countered that Sam's message in giving the ring was to show "the importance of love" and "making the most of it", adding that there's no reason their engagement had to keep them from helping their friends. "It is life affirming for us to get engaged. Not just for us, but for everyone we know," he insisted and got back onto his knee with a new proposal. This time, she looked like she would say yes, but then the phone rang.

Noah was furious with his father for badmouthing him to Paloma, but then Sam started laughing. "You're too easy, Noah," he chuckled, revealing he'd known his son was hiding the entire time he was ragging on him. Paloma was relieved, but didn't see the humor. The chief said it was his way of teaching them a lesson, since Noah wasn't supposed to be at the station. "Instead of getting angry with you, I thought I'd have a little fun," he said and proceeded to retract the bad things he'd said of his son. Paloma was even more annoyed to learn that pranks were a family thing, but she finally smiled. When she left the guys to themselves, Sam turned serious. He gave Noah something special and told him it was "time," sharing words of wisdom Grace would have given if she were alive. When Paloma returned, Noah stunned her by showing her what his father had given him.

Another guard gave Luis trouble for being a cop turned bad, but it turned out the guard was the dirty cop, which was why Luis had gotten him fired from the force. The guard got his revenge by letting two goons into Luis' cell. Luis took quite a beating, but the guard called them off before they killed him. He was looking forward to seeing Luis put to death in front of his family. When Fancy visited, Luis denied the fight and then begged her not to talk to the warden. Fancy filled him in on the DNA they'd gotten, which didn't have a match in the criminal databases. She was confident they'd find the match in the hospital databases, but Luis wasn't happy to hear she was investigating without Sam. He insisted the blackmailer was too dangerous for her to be working alone. She countered that the Chief would throw time—consuming legal hurtles in her way, which Luis didn't have the time for. He felt helpless, insisting he should be the one by her side. She promised she'd clear his name so she could spend the rest of her life with him.

Chad showed up to visit Whitney and Miles, who were getting settled in Whitney's old room. She insisted she was too angry to discuss the future and that it was "far too late" for apologies. "We are no longer a family," she said and told him he "should have thought of Miles before you slept with Vincent." He insisted he wasn't gay and that he loved her, but nothing he said got through. She referenced their rocky start — from the triangle with her sister to his secret ex-wife Latoya — and then his recent affair. She insisted she'd finally gotten the message: "We were never meant to be together to begin with." Later, she said, "Love shouldn't be this hard, okay? And if it is, something's wrong." Tears fell as she calmly gave him her wedding ring but she pulled them back and sent him away. "You just know that I love you and I always will," he said before leaving.

Father Lonigan urged Theresa to find a way to tell Ethan the truth about Little Ethan, especially as the blackmailer and Julian were holding it over her. As Theresa bemoaned having to trust Julian, she noticed Ethan standing there. Ethan put her in an apologetic panic by saying he'd heard "enough", but as always, he hadn't. He only heard enough to know Julian knew the secret. That hurt his feelings, but he said he understood that he had to tell Julian because the secret was about Little E. "No father should be should be kept in the dark about what's going on in his child's life," he said. After they talked about smarmy Julian, Ethan revealed he not only loved Little E like his own, but sometimes also felt like Little E was his own. After asking her to trust him with the truth soon the way he trusts her, Ethan left. Lonigan chided Theresa for not telling him the truth, but she was too afraid she'd lose Ethan to his anger if she told him.
